What you’ll do:
The Crisis Transition Navigator (CTN) works with Non-Title XIX, and Title XIX-Non-SMI Adults, who have received services from the Maricopa County crisis system. The Crisis Transition Navigator serves as a bridge to ongoing services by coordinating and monitoring RBHA/Community-based resources and facilitates delivery of these necessary services and supports. The goal of the CTN is to provide individuals who are voluntary and able to give informed consent, assistance in accessing post-crisis services to assist with food, housing, financial stability, and identifying support systems.
Essential Responsibilities:
Meets with consumers of crisis services in the community as referred by specified crisis providers. Completes AHCCCS applications and/or collaboration with DES offices for persons that meet AHCCCS eligibility criteria. Assists in mapping out the most appropriate next steps for the individual who may need assistance in accessing various other providers or community based services after their initial crisis has passed, including development of a Crisis Navigation Plan (CNP). The plan will be self-directed by the recipient and coordinated with the CTN. The CTN will follow up and assist with implementation of the needs identified in the CNP. Will review and monitor recipient progress.
Develops and collaborates with an extensive network of Community Based and RBHA resources. The CTN must know how to access various social/behavioral health community based services that can include but are not limited to: food, clothing, housing, support services, and Outpatient Crisis Stabilization/Transition services. This can also include making necessary appointments, assisting with scheduling or arranging SMI evaluations and Outpatient Stabilization/Transition services when applicable.
Maintains accurate and thorough documentation in progress notes and in the client's Crisis Navigation Plan (CNP).
Serves as a short term recovery coach, helping the participant develop and implement self-directed recovery plan, sharing experience, providing support, teaching self-management techniques and reassurance.
Requires competency (via specialized training) in the following areas:

*In Home Safety
*AHCCCS eligibility/Affordable Health Care Act
*SMI eligibility
*Crisis System
*Other identified peer specialist and system training
Additional Responsibilities may include:
Facilitating appointments with treatment providers for behavioral health assessments when applicable.
Coordinate post-crisis/stabilization transportation.
Assisting recipients in obtaining Birth Certificates and/or other forms of acceptable identification if needed.
Identifying transportation options.
Referral for SMI evaluations when criteria are met.
Coordinating housing with community shelters, apartments, and housing facilities.
Recommends to the Program Coordinator or Manager changes that would improve service delivery. Select and develop one specialized function related to the development of department services.
Participates in agency fund raising activities as requested by management.
Position requires driving of company van and/or personal vehicle. Must maintain a valid Arizona driver's license and appropriate liability insurance at all times. Also requires eligibility for liability coverage with EMPACT’s insurance carrier.
What you’ll provide:

Two years of prior experience preferred.
Effective communication skills evidenced by a non-judgmental approach.
Effective listening and positive interaction; thorough understanding of the role of family support at each stage of the recovery process.
Must have ability to effectively interpret and implement complex treatment plans based upon a wide knowledge base of available community services.
Must have Fingerprint clearance card or be eligible to obtain one.

The High Needs Case Manager works with children and families that require a high level of service provision, contact, and coordination of treatment. This position provides direct service crisis intervention, support, transportation, and education to families and children, independently of, or in conjunction with, other in-home client services. Engage and invite natural supports as identified by family, youth, and adults. Make referrals to collaborating partners and other community services, as client needs indicate.
Deliver at least 40 hours of work per week, with a goal of 30 hours of direct client contact, which includes case management as needed. Ensure meeting clinical documentation standards established monthly/annually. Complete all tasks agreed upon by the timelines given. Must be able to work a flexible schedule based on the needs of the children and families, which may include evening or weekend hours when needed. Evening and weekend hours may be required to accommodate the schedules of families, youth, and adults. Attend meetings and training as requested.

The Family Support Partner models the skills necessary to maneuver within the behavioral health system and other community programs, thus serving as a system guide and advocate for the parent. Provide direct support to families as an active member of the Child and Family Team. Encourage family members to move toward self-advocacy in the attainment of their own, culturally specific life goals for themselves and their children and help them develop the skills to do so. Work with the family to gather information regarding their strengths and needs, as well as provides information, which enables the family to make decisions in the child and family planning process and beyond. Provide information and connections to community resources, and helps the family navigate through the processes of the education, behavioral health, juvenile justice, and social services systems. This position is full time and must have the flexibility to work an occasional weekend day or evening to meet a client's needs.
What you’ll provide:
Minimum of a High School Diploma or GED.
Experience as a parent, foster parent, adoptive parent or a caretaker of a Child with Behavioral Health needs and experience with the children's services systems. The child MUST have lived with you.
Organizational and Time Management skills to work effectively in this busy position.
Computer literate, good customer service skills, basic typing skills.
Able to work professionally and effectively with others from diverse ethnic and cultural groups.
Good written and verbal communication skills.
Possess a valid ARIZONA driver's license w/no major infractions for past 39 months.
Must have a Fingerprint Clearance Card or be eligible to obtain one.
Bi-lingual (Spanish) preferred.
Must be 21 years of age or older at time of employment.

POSITION SUMMARY:Under the supervision of the Registered Nurse, position provides direct patient care to patients as assigned, maintains a safe and efficient working and treatment environment per facility policies and procedures, and communicates effectively with the treatment team to ensure safe, quality care is provided to all patients.
PRIMARY DUTIES:
DIRECT PATIENT CARE
* Carry out all aspects of patient care and other assignments as directed by the Charge Nurse.
* Observe whether specialized services may be needed and communicate to the Charge Nurse/RN.
* Perform ongoing observation of the patients and milieu to ensure active treatment is provided and patients are participating.
* Respond appropriately and timely to patient requests.
* Report changes in patients' condition or behavior to the Charge Nurse/RN, including agitation, high-risk behaviors, changes in vital signs, and eating, sleeping, and toileting patterns
* Intervene appropriately with patients experiencing a behavioral crisis situation, i.e., suicidal, threatening behavior, elopement and/or other situations, using approved interventions.
* Initiate and/or participate in provision of emergency medical care when indicated.
* Provide support and direct assistance to patients who are unable to attend to their activities of daily living by assisting them with meals, daily grooming, ambulation and toileting as needed.
* Participate with the treatment team in implementation of the individualized plan of care. Report factors that may impede successful accomplishment of the treatment plan to the Charge Nurse/RN.
* Provide patient educational information for identified learning needs.
* Facilitate educational/activity groups as assigned or when indicated using approved materials. Recommend alternative activities based on patient functioning level and treatment plan.
* Ensure compliance with infection control protocols and/or policies by patients, visitors, and staff.
SHIFT SUPPORT
* Review assignment sheet and program designated communication tools each shift, i.e. communication log, bulletin boards, etc.
* Facilitate a therapeutic and safe environment.
* Remain on-duty until coverage for subsequent shift arrives on unit and has received verbal report.
* Ensure all patient valuables retained at the hospital during inpatient stay are recorded and stored per policy.
* Ensure patients are discharged with all personal belongings, medications, prescriptions, discharge, and referral information.
* Perform environmental and safety rounds and complete environmental check sheet as assigned. Act to mitigate risk and report identified problems to the Charge Nurse/RN.
* Comply with National Patient Safety Goals and provide immediate feedback if non-compliance is identified.
CUSTOMER INTERACTION
* Interact with patients, family members, external entities and internal staff professionally and respectfully.
* Work collaboratively with all treatment team members to create a treatment environment that supports the facility's mission to provide exemplary patient care and customer service.
* Act to preserve patient and family dignity and safety.
DOCUMENTATION
* Documentation for medical records and reports is timely, legible, thorough, and in required format.
